Continuum of Care
A unique aspect of Crotched Mountain Specialty Hospital is its ability to provide a seamless continuum of care from post hospital to acute to sub-acute to skilled nursing. Patients can be admitted at post hospital, acute and sub-acute levels of care and remain at one facility for all their rehabilitation and medical needs until they are ready to return home, enter a community program or discharge to a long term care facility.
This distinctive approach to rehabilitation ensures a uniformly rewarding patient and family experience at every stage of inpatient treatment. By avoiding the need to transition to a new provider and facility at different phases of recovery, patients receive consistent treatment in familiar surroundings with interdisciplinary rehabilitation teams who understand their needs.
And when patients return home, a full complement of support services are available through an organization they have come to know and trust.
