Crotched Mountain Community Services offers specialized programs for individuals with disabilities throughout New Hampshire. From accessible recreation to adult residential living to community-based day services to case management, our goal is simple: to help people with disabilities remain active, valued participants in the community and build lives of maximum independence.
Crotched Mountain Community Care
Crotched Mountain Community Care (CMCC) provides person-centered case management services to individuals on the Choices for Independence (CFI) Medicaid waiver program throughout all of New Hampshire. An award-winning, proven leader in the case management field for over 30 years, CMCC is committed to helping people remain independent, safe, and healthy in their own homes.
Serving nearly 900 clients all across the Granite State, CMCC Case Managers coordinate in-home services like personal care and nursing, provide access to resources for medical equipment and home modifications, and maintain an ongoing relationship through regular home visits and phone calls.
CMCC has administrative offices in Portsmouth, Manchester, Greenfield, and Whitefield, and home offices spread throughout the state.
Crotched Mountain Accessible Recreation and Sports
Crotched Mountain Accessible Recreation and Sports (CMARS) provides exciting outdoors experiences for people of all abilities, like kayaking, skiing, cycling, and more.
CMARS is available to everyone in the community–children and adults alike–delivering an adaptive recreation menu that spans casual outings to varied degrees of instruction. Our programming is customized and goal-oriented, taught by certified and licensed recreational therapists.
CMARS staff and volunteers are trained to work with individuals with varied physical, developmental, or emotional disabilities including autism spectrum disorders, brain injury, cerebral palsy, emotional disorders, neurological impairments, seizure disorders, sensory impairments, spinal cord injury and orthopedics. Additionally, CMARS provides recreation programming for senior citizens.
Refurbished Equipment Marketplace
From power wheelchairs to patient lifts to speech devices and everything in between, the Refurbished Equipment Marketplace (REM) is your destination for high-quality, pre-owned medical equipment up to 90% off the MSRP! REM takes in donations of medical equipment which are then cleaned, serviced, and tested by our experienced, qualified techs. REM also offers repairs services and a large selection of parts.
The REM inventory is available to view and purchase at any time on the REM website. Customers are also encourage to browse the inventory in person at the REM showroom at: 57 Regional Drive Suite #7a, Concord, NH. Adult Community-Based Services
For adults with disabilities, Crotched Mountain’s community living and community participation opportunities offer individualized, relationship-based residential and day experiences.
Crotched Mountain’s ISOs (Individualized Service (Options) are life-sharing homes, where participants live with a home provider in their home (or vice versa!). Participants and providers are choosing to create a life together, based on existing relationships and compatibility. Community residences are for individuals who require increased supervision and support, these residences allow community living for participants who might otherwise be in a more restrictive setting.
Community Participation Services
Crotched Mountain’s day programming focuses on: life and vocational skills, social opportunities, and recreation. All of participants have one-to-one staff, allowing for extraordinary flexibility. The daily schedule is as nimble as the individual wants it to be.
